Defect Tracking for Acceptance Testing

Record UAT defects with severity, supporting evidence and links to test executions. Give testers and reviewers a shared view of issues that affect acceptance.

Make a defect reproducible

Describe the starting state, the steps taken, the expected outcome and the actual result. Include a screenshot or supporting document when it explains the issue. Avoid uploading passwords, access tokens or unnecessary personal information as evidence.

Keep the testing context attached

Link the defect to its test execution so the team can understand where it was found. A failed business workflow may affect several requirements; connected test cases help reviewers discuss that impact without reconstructing the history from separate messages.

Agree severity and review the outcome

Use severity to communicate the impact of an issue and keep its status current as the team works through it. An issue that blocks a core business process needs a different acceptance discussion from a cosmetic problem with a practical workaround.

Retest before the acceptance decision

After a fix, repeat the affected scenario and record the new outcome. Check related workflows when the change may affect them. At sign-off, review unresolved defects and explain any accepted limitations instead of treating the total pass count as the whole decision.
